Plan (Scribe) → Build (Builder) → Review (Overseer)

- Planning produces a Phase folder on disk
- Building modifies code and updates progress
- Review either declares the Phase complete OR adds follow-up steps and sends back to Build
No Phase is complete without Overseer approval.
